Rhinoplasty, or rhinoplasty, performed to beautify the nose, is still among the most preferred surgeries. Aesthetic and Plastic Surgeon Op. Dr. We asked Metin Kerem.

I want to have rhinoplasty, how do I know if I am a suitable candidate?
First of all, when you look in the mirror, honestly ask yourself if you like the shape of your nose. If the answer is yes, consult a specialist, that is, get the opinion of a surgeon. A surgeon who is honest and will guide you correctly always tells the truth. If nose surgery will be unnecessary for you, maybe he can direct you to different small interventions or not touch your nose. If it is a question of achieving a natural, beautiful and quality change in your nose shape, your surgeon will already begin to guide you on your rhinoplasty journey. Besides, if you are having difficulties with breathing, you are already a good candidate for this condition. For example, if you have bony curvature, this problem will disappear when we bring the nose to the midline. Or you have a very arched and large nose; downsizing is possible. In fact, although it is not seen in Turkish patients, we have the opportunity to enlarge the noses, especially in patients of ethnic origin, because their noses are very small. In other words, if the patient is a suitable candidate, it is possible to beautify a nose shape in many different variations.

The most curious problem of everyone; swelling after rhinoplasty. How long does the nose stay swollen?
Generally, the nose continues to be swollen for up to 6 months, but of course, the most prominent swelling is the process up to 1 month. Afterwards, the swelling gradually decreases and the subsequent swelling process is not as obvious as the first month. After removing the nose plaster, patients think that they will see immediate results, but this is a misconception. Just as there is edema after a trauma in any part of our body, we also encounter edema after rhinoplasty. And this situation does not pass immediately. The tip of the nose remains swollen for a while, and the lines of the nasal bridge are not yet clear. However, the patient feels the change, especially in the first 3 months, that little swelling goes down every week.

Do you have a very painful process?
No, since the techniques have advanced so much and technology has developed so much, there is an incomparable improvement with the old nose surgeries… I prevent the patient from feeling pain when he wakes up after the operation, by applying local anesthesia to the operated area after the operation. Since he does not feel pain at that moment, he does not feel pain in the rest of the psychological process. In addition, since we use a technique that traumatizes the nose as little as possible, it is almost impossible for the patient to feel pain compared to previous techniques.

Should we be afraid of not being able to breathe after surgery?
Of course, you will have difficulty in breathing because there are tampons in the nose for the first 3 days, you will breathe more through your mouth, which will cause dry mouth, but this is valid for a very short time. You can breathe easily after the tampon is removed. Even if there are air channels in the tampons, patients usually complain of not being able to breathe much for the first 3 days. But after that, of course, this problem goes away.

What about skin care practices during the healing process?
After the surgery, the skin may be oilier for a while. So just keep moisturizing your face for a while after the plaster is removed. After 1 month from the surgery, you can apply peeling and mask once a week to ensure the skin oil balance.

What do you think an ideal nose should be like?
Of course, both functional and aesthetic. So, in my opinion, it should be both breathable and have a beautiful appearance. What I mean by a beautiful look is that it is a natural look. In other words, when someone looks at it, it should not be understood that you have had a nose surgery, but you should feel that there is a light and a change in your face.

Do you generally prefer open or closed rhinoplasty technique?
I generally prefer closed technique in primary cases, but more open technique in secondary revision patients. Of course, this varies from patient to patient. In other words, I have patients who have revision and prefer closed rhinoplasty technique.

Let’s say I decided to have a nose surgery, how should I make my choice?
First of all, you should have a good research process. Make sure your surgeon is ‘European Board Certified’ (EBOPRAS certified). I always believe that the best advertisement is testimonials. That’s why, somehow, try to reach the former patients of the doctor you want to have surgery, examine their comments. If you have several different surgeons in mind, check them all out. They will offer you different perspectives on your surgery. The perception of beauty is personal, it would be best for you to proceed with the surgeon who has the closest point of view to you.

Is it appropriate to have rhinoplasty in summer?
Of course, it is convenient, especially since it usually coincides with the summer holidays of foreign patients, this group prefers the summer months. But of course, if you ask me; It makes more sense to apply it in the winter or spring season. It is really difficult not to wear sunglasses in summer and we recommend our patients not to wear glasses for at least 1, but preferably 2 months after surgery. In addition, the air temperature overwhelms people and it can be difficult to deal with hot weather during the surgery process. That’s why spring months should be the most preferred period in my opinion.
